An individual plan of care is developed for each participant based on his or her unique needs, including:
- Nursing and personal care, including medication monitoring
- Physical therapy
- Occupational therapy
- Speech therapy
- Maintenance therapies
- Alzheimer’s Program
- Case management
- Stimulating group activities
- Psychological counseling
- Nutritional counseling
- Respite for caregivers
- Support groups for clients and caregivers
- Family Caregiver Support Program
- Hot, nutritious lunch served daily
- Wheelchair-accessible transportation
- Information and referral
Costs are determined on an individual basis, and may be covered by Medi-Cal and other health insurance plans. Sliding scale rates based on income can be arranged.
